What Is Lichen Planus?

While we are still uncertain what exactly causes lichen planus, it is believed to be an autoimmune condition where the body’s immune system mistakenly attacks cells in the skin and mucous membranes. The condition can develop suddenly and may affect various parts of the body, including the mouth, scalp, and nails. While some cases resolve on their own, others may require medical intervention to manage symptoms.

Lichen planus is an inflammatory condition that can affect the skin, hair, nails, and mucous membranes. At AboutSkin Dermatology and Aesthetics, our Board-Certified Dermatologists provide comprehensive evaluation and evidence-based treatments for managing this chronic condition.

What Causes Lichen Planus?

Several factors may contribute to the development of lichen planus:

  • Immune system dysfunction
  • Viral infections
  • Certain medications
  • Genetic predisposition
  • Stress
  • Environmental triggers
  • Underlying medical conditions
  • Metal dental fillings (in some cases of oral lichen planus)

While managing stress and maintaining overall health can support immune system function, these measures alone may not prevent or resolve lichen planus due to its autoimmune nature. The condition’s development and progression are primarily influenced by internal immune system responses rather than lifestyle factors. This autoimmune activity can persist regardless of general health habits, and the condition may become more complex over time if the underlying immune response is not addressed through appropriate medical intervention.

Symptoms and Impact

Lichen planus can present differently depending on the affected area. On the skin, it typically appears as purple, itchy, flat-topped bumps. These bumps often have a shiny or glazed appearance and may develop white lines called Wickham’s striae on their surface.

Oral lichen planus can cause burning sensations, sensitivity, and discomfort in the mouth. It may appear as white, lacy patches on the inner cheeks, gums, or tongue. In some cases, painful ulcers may develop.

When lichen planus affects the scalp (lichen planopilaris), it can lead to inflammation around hair follicles, scarring, and permanent hair loss if left untreated. Nail involvement may result in thinning, ridging, or splitting of the nails.

What Lichen Planus Treatments Are Available?

At AboutSkin Dermatology and Aesthetics, we offer several treatment options for managing lichen planus. Our Board-Certified Dermatologists will help determine which treatments are most appropriate based on the location and severity of your condition.

Topical Medications

Prescription corticosteroid creams, ointments, or gels can help reduce inflammation and relieve itching. Other topical medications that affect the immune response may also be prescribed.

Oral Medications

For more extensive cases, oral medications may be recommended:

  • Systemic corticosteroids can help control severe outbreaks.
  • Immunosuppressive medications may be prescribed to modify the immune system response.
  • Antihistamines can help manage itching and discomfort.

Phototherapy

This treatment uses controlled ultraviolet light exposure to help reduce inflammation and manage symptoms. Different types of phototherapy may be recommended based on your specific case.
